Dr. Jonathan S. Rubens
Chief Medical Officer, ActiveHealth
Jonathan S. Rubens, MD, MHPE, FACEP, FAAEM, CHIE is Chief Medical Officer of ActiveHealth Management. He joined ActiveHealth® in 2011 and his responsibilities have included clinical leadership, evidence-based research and content development, education, quality monitoring, and informatics and outcomes functions. As CMO he heads a team of physicians focused on population health and leads the enterprise strategies for the application of clinical tools and analytics to improving individual and population-based health outcomes. He is board-certified in Emergency Medicine, and a fellow of both the American College of Emergency Physicians and the American Academy of Emergency Medicine. Dr. Rubens earned his Bachelor of Arts degree from the University of Pennsylvania and his Doctor of Medicine degree from Wake Forest University. He went on to study Health Professions Education and earned his Master’s Degree from the University of Illinois at Chicago. Dr. Rubens brings over 25 years of experience as a physician leader in both academic and community settings. Originally from New York City, Dr. Rubens resides in North Carolina.
Ms. Nancy Slattery
Clinical Team Lead, ActiveHealth
Nancy Slattery is a Clinical Team Lead at Active Health Management. She holds a RN BSN degree from George Mason University, and has been a member of the ActiveHealth® team since 2003. Nancy currently functions as a Team Lead for the High Point, NC Case Management team, however she is a subject matter expert for all ActiveHealth Case Management and Behavioral Health teams, regarding clinical content and work flows. Nancy is passionate about mental health issues. She coordinates care for behavioral health members and provides consults to those members who have complex needs. She collaborates with Clinical Supervisors, Medical Directors, Quality Management, and the Product team, to streamline our organizational processes and procedures.
Recently, Nancy was an integral member of a multidisciplinary Behavioral Health team at ActiveHealth, that revised both the ActiveHealth Safety Plan, and the Depression Assessment Tool. She then independently developed, and delivered, the finalized work flow to all clinical teams. Nancy has 42 years of healthcare experience. She began her nursing career on an inpatient Medical unit, where she was a Clinical Manager. Later, she started working with Substance Abuse and Mental Health patients, before moving into Case Management.
Dr. Gianna Bryan
Clinical Pharmacy Manager, ActiveHealth
Gianna Bryan, Pharm.D., BCGP is the Clinical Pharmacy Manager overseeing the Medication Therapy Management (MTM) for ActiveHealth Management. She has numerous years of experience in pharmacy practice in diverse settings including clinical teams and clinics, long-term care and operations. Her current responsibilities include development and administration of MTM services for a large state client. She also serves as medication subject-matter expert for her team and is a contributor to content development for ActiveHealth®.
Dr. Bryan holds a Doctorate in Pharmacy Practice from Campbell University, Board Certification in Geriatric Pharmacy, and numerous practice and disease-state certifications. She has authored multiple publications for pharmacy practice journals and provided presentations for various educational forums.
