Our health assessment adds another layer of understanding about each member in our health and wellbeing program. The assessment is easy to complete and the data is incorporated in our technology platform to help identify risks, gaps in care and other important opportunities for health improvement. With integration into the MyActiveHealthSM platform, the assessment can be completed from a smartphone and referenced easily whenever necessary.
With biometric screenings, our team is able to deliver an annual health screening, with important baseline data that might indicate risk factors that may not otherwise be identified, and also provide participants more relevant, personalized strategies for improving their overall health.
The member engagement platform also provides access to educational resources and tools like a personal health record, preferred provider contact information and insights from our patented technology platform to help participants conveniently manage their health and wellbeing.
ActiveHealth offers several modalities of coaching, including digital coaching, group coaching and one-to-one support, to provide every member with the right interactions to help meet their goals. Our coaches get to know each individual personally, so they can better understand what motivates them, and maps out effective strategies for better health and wellbeing.
Every population is different, so we use years of experience and research to develop strategies that will support a culture of wellness for your unique organization. We can help you decide on incentive design, type and execution variables (e.g., participation-based versus outcomes-based) as well as track performance. Once an incentive strategy is in place, we also provide tools to drive individual engagement, like an online rewards center integrated with the MyActiveHealthSM platform to track and help guide participants.
